It involves completing a questionnaire based on your answers to questions about your health needs and lifestyle preferences.

You will also have an opportunity during this assessment to talk about any concerns you may have regarding living at home alone; for example, if you feel depressed or anxious about going out alone because of having a severe disability or long-term illness. This might help inform what type of support could be offered under the scheme such as befriending services where volunteers visit people who don’t feel comfortable going out alone due their illnesses/disabilities
